INTERNAL NOTIFICATION SYSTEM

Our company has established an internal whistleblowing system and accepts reports in accordance with Act No. 171/2023 Coll. on the protection of whistleblowers. Below you will find all the reporting channels, together with a description of how to submit and track a report. Only the designated person may access a report once it has been received. All information provided is therefore completely secure.

The designated person responsible for receiving, investigating and further handling reports is: Ing. Jan Kadlec, tel: +420 605 266 804, email: jan.kadlec@usspa.cz. These contact details are not intended for the submission of reports. Reports may be submitted using the methods set out below.

OPTIONS FOR SUBMITTING A NOTIFICATION

In accordance with the Act, we allow reports to be made in writing, verbally or in person. 


The whistleblower may submit a report via the online form available at www.nntb.cz/c/buh9ebn0 or on the website www.nntb.cz – the ‘report’ icon in the top bar – My Organisation and enter the code: zsvicdzbz7


The relevant person will also accept reports verbally during a face-to-face meeting. A request for a meeting can be arranged by emailing jan.kadlec@usspa.cz or by telephone on +420 605 266 804. The notifier will be given the opportunity to make a verbal notification within a reasonable period, but no later than 14 days from the date on which the notifier made the request.

WHAT CONSTITUTES A NOTIFICATION UNDER THE ACT

If you report conduct which:

  1. constitutes a criminal offence,
  2. constitutes an administrative offence for which the law prescribes a fine with an upper limit of at least 100,000 CZK,
  3. breaches the Whistleblower Protection Act, or
  4. breaches any other legal regulation or European Union regulation in the areas specified by the Act (e.g. financial services, corporation tax, consumer protection, environmental protection, etc.)

DATA PROTECTION

The relevant person is obliged to maintain an electronic record of details of reports received and to retain reports submitted via the internal reporting system, together with any documents relating to the report, for a period of 5 years from the date of receipt of the report. In the case of reports submitted via the internal reporting system, only the designated person shall have access to the records, the documents relating to the report and the retained reports.

EXTERNAL NOTIFICATION CHANNEL
